PAYROLL CARD E-SIGN CONSENT & ELECTRONIC DELIVERY AGREEMENT
PLEASE READ THIS INFORMATION CAREFULLY AND PRINT AND/OR DOWNLOAD THIS DISCLOSURE TO RETAIN A COPY FOR YOUR REFERENCE
For purposes of this consent, “we”, “us”, and “our“ refers to SchoolsFirst Federal Credit Union (SchoolsFirst FCU), and “you” and “your” refers to each and every person who signs electronically.
In order to establish your Payroll Card Account with SchoolsFirst FCU, your consent to conduct business electronically is required.
Pursuant to the federal Electronic Signatures in Global and National Commerce (ESIGN) Act, we must obtain your consent prior to conducting business with you electronically or delivering any communications, documents, agreements, and other legal disclosures to you in an electronic format. The E-Sign Consent and Disclosure Agreement documents your consent and explains your rights to electronically receive disclosures and notices relating to the Payroll Card Account you are applying to open with us online. These communications may include, but are not limited to: the E-Sign Consent and Electronic Delivery Agreement, the Pre-disclosure for Payroll Card Accounts, the Payroll Card Account Disclosure and Agreement of Terms and Conditions, our Privacy Policy, any future changes in terms, notification of when your written history of Account transactions/eStatements are available via Online Banking, subsequent disclosures required by law or regulation, and other communications relating to your Payroll Card Account.
Note: Communications, disclosures, agreements, and other notices provided to you electronically are considered “in writing” and are available to you in a form that you may keep by either printing or downloading the documents.
Requirements to Print and Download Documents
Equipment Requirements: To receive disclosures electronically, you must use a computer that meets the minimum hardware and software requirements, as well as browser and other requirements.
Hardware and Software Requirements:
- To print, you must have a printer connected to your computer
- To download electronic communications, your computer must have sufficient hard-drive space to download store and view information
- A modem or other device allowing connection to the Internet
- An Internet browser that can support the service
- An established e-mail account and address, and the ability to access attachments to e-mail
- An up-to-date version of Adobe® Acrobat ®Reader or Adobe Acrobat Reader browser plug-in. If you do not have this software, you can download a free up-to-date version at www.adobe.com
Browsers and Other Requirements:
Use of SchoolsFirst FCU's Online Banking system requires updated versions of Microsoft Internet Explorer, Firefox, Safari or Google Chrome for both PC and Mac. Minimum Browser Versions Supported are:
- Microsoft Internet Explorer 11 (PC)
- Firefox 40 (PC & Mac)
- Safari 8 (Mac)
- Google Chrome 44 (PC & Mac)
Modification of browser security for items such as “cookies” may be required.
Requesting Paper Copies of Electronic Records
You have the right to request a paper copy of any document provided to you in electronic format at no cost to you, by notifying us at info@schoolsfirstfcu.org, by calling us at 855.312.0029, or by writing to us at SchoolsFirst FCU, Attn: Debit Card Services, P.O. Box 11547, Santa Ana, CA 92711-1547. If you request to have a paper copy sent to you, other than through the regular U.S. mail, you may be charged a fee.
Withdrawing Consent to Receive Notices Electronically
You may withdraw your consent to receive future notices and disclosures in electronic format, at no cost to you, by notifying us at info@schoolsfirstfcu.org, calling us at 855.312.0029, writing to us at SchoolsFirst FCU, Attn: Debit Card Services P.O. Box 11547, Santa Ana, CA 92711-1547, or visit Online Banking to change your preference for eStatements and eNotices. If you withdraw your consent, the legal validity and enforceability of prior electronic communications will not be affected.
Consequences for Withdrawing Your Consent to Receive Notices Electronically
If you withdraw your consent, we will no longer send notices or disclosures to you electronically, this includes notifications about your written history of Account transactions or eStatements, which are available for viewing through Online Banking. The only documents that we will automatically generate and send to you by regular U.S. mail will be those that we are required to send to you by law or regulation, such as a Change in Terms notice. By withdrawing your consent to electronic notices, you acknowledge and understand that you will not receive monthly or quarterly periodic statements through the mail, but you may continue to monitor and access your Payroll Card Account balance and a 12 month history of your Account transactions or eStatements by logging on to Online Banking at www.schoolsfirstfcu.org. You may also obtain information regarding your Payroll Card Account balance and a history of your Account transactions by calling the Credit Union at 855.312.0029.
Updating your Records
It is your responsibility to provide us with an accurate and complete: e-mail address, contact information, and other information related to any agreements and disclosures on your Payroll Card Account. It is your responsibility to maintain and update any changes promptly, should this information need to be changed. We are not responsible for any lost or un-received emails or documents, due to incorrect or outdated e-mail information on file. You can update your e-mail information by notifying us electronically at info@schoolsfirstfcu.org, by calling us at 855.312.0029, writing us at SchoolsFirst FCU, Attn: Debit Card Services, P.O. Box 11547, Santa Ana, CA 92711-1547, or by visiting one of our branches.
Consent
By selecting this box and moving forward with your application, you acknowledge that you have read and understood the E-Sign Consent and Disclosure Agreement, and you affirmatively consent to conduct business electronically with SchoolsFirst FCU regarding your Payroll Card Account.
